Why can lactose cause diarrhea in some people?

This is one of the hallmark symptoms of lactose intolerance, and it happens because of osmosis and bacterial fermentation working together.
Step 1: You drink milk
Milk contains lactose.
Normally, the enzyme lactase in the small intestine breaks lactose into:
